Dialysis Social Worker - Patient Advocate & Care Coach
U.S. Renal Care
U.S. Renal Care is seeking a Licensed Master Social Worker to provide timely psychosocial services to dialysis patients and their families. You will support education, advocacy, and resource coordination within the clinic setting. Responsibilities include coordinating with the interdisciplinary team, completing initial assessments and KDQOL, and participating in QAPI activities to improve patient outcomes and satisfaction.
