Job TitleBased on Experience: *Range for 2 year degree $27.29-$43.40 *Range for 4 year degree $28.30-$44.97Essential FunctionsBuilds relationships with patients and coordination with primary care team, specialty care teams, and inpatient facilities.Develops care plans for patients with Program.Monitors HIT tools and reports to identify gaps in care and needed services for patients within program.Addresses medication alerts, hospital admissions and discharges, including emergency visits.Monitor and report performance measures and outcomes.Skills AbilitiesAbility to communicate clearly and effectively with co-workers and patientsAbility to complete competency CPR and First AidAbility to prioritize and compete multiple tasks simultaneously.Effective oral and written communication skills.Working ConditionsPossible exposure to inside environmental conditions.Exposure to distressed patients, families, or visitors.Physical DemandsRequires standing, sitting or walking for up to 8 hours or longer per day.The ability to lift up to 50 lbs.QualificationsActive Missouri RN licensePreferred QualificationsClinical experience preferred but not requiredPCHH and/or Chronic Care Management nurse experience
