About the Role
A leading regional healthcare provider known for patient-centric care is seeking a Hospice Registered Nurse Case Manager to join its interdisciplinary hospice team. The organization is a respected community hospice that serves patients and families in homes, assisted living communities, and skilled nursing facilities. Full-time and per diem schedules are available with a steady weekday routine and a shared on-call rotation. This role suits an RN who brings calm clinical judgment, strong symptom management skills, and genuine compassion to end-of-life care.

The hospice team works closely together, with physicians, social workers, chaplains, aides, and volunteers all supporting each patient and family. New nurses complete a structured orientation that includes shadow visits and documentation training, and leadership keeps caseloads and on-call expectations sustainable so nurses can give each family the time and attention they deserve.

Core Responsibilities

• Manage a caseload of hospice patients across home and facility settings
• Complete admission, routine, and recertification visits and comprehensive assessments
• Develop and update individualized plans of care with the interdisciplinary team
• Provide expert pain and symptom management in partnership with the hospice physician
• Educate patients and families about the dying process, medications, and comfort measures
• Coordinate care with aides, social workers, chaplains, and volunteers
• Document visits accurately and on time in the electronic medical record
• Participate in weekly interdisciplinary group meetings
• Take part in the on-call rotation and respond to urgent needs
• Support families with dignity and compassion through end-of-life care
• Provide guidance and support to families at the time of death and coordinate post-death visits
• Mentor hospice aides and verify that aide care plans are followed

Required Qualifications

• Active, unrestricted RN license in the state of practice or an eligible compact license
• Current BLS or CPR certification
• One year or more of RN experience; hospice, home health, oncology, or palliative care experience preferred
• Valid driver license, reliable transportation, and current auto insurance
• Strong communication skills and emotional steadiness in sensitive situations
• Ability to work independently and manage time across a territory
• Knowledge of hospice regulations and Medicare hospice benefit preferred
• Commitment to patient dignity, privacy, and family-centered care

Benefits & Pay
Pay: $36.00 - $44.00 per hour plus on-call pay, based on experience and schedule.
