Title: Family Nurse Practitioner or Physician Assistant (FNP, PA) Reports To: Director of Primary Care & Quality Status: Full-time, Exempt Location: Antioch, CA Pay: $125,000 - $150,000 per year (depending on experience). Additional $10,000 sign‑on bonus for bilingual Spanish/English fluency or $5,000 for non‑Spanish speakers. It began in response to alarming differences in mortality and illness rates among African American babies compared to other children. The organization now serves communities in Oakland, Richmond, Bay Point, and Antioch with a multicultural, bilingual staff of passionate family service professionals. Position Summary The Family Nurse Practitioner (FNP) or Physician Assistant (PA) will provide direct patient care and perform administrative duties as designated by the Director of Primary Care & Quality. The overall direction for all patient‑care activities is provided by a Medical Doctor; administrative supervision is given by the Director of Primary Care & Quality. Qualifications Valid California license to practice as an FNP or PA with authority to prescribe and furnish medications. Master’s degree or higher. Certificate of completion from an accredited nurse practitioner program. Board certification required. Current BLS certification. Current DEA certification. Valid California driver’s license. Proof of auto insurance. Knowledge of appropriate patient‑care rationale. Ability to maintain good interpersonal relationships with co‑workers and health team members. Proficiency in Microsoft Office, Microsoft Access, data entry, and business email. Passed physical exam and TB test. Primary Care Provider Responsibilities Provide complete patient history, physical examinations, diagnosis, and treatment plans for primary, secondary, and tertiary care services in compliance with standardized protocols. Communicate care plans and results to patients or guardians and answer questions; organize and deliver health education. Coordinate referrals to specialized health care and public agencies. Review patient files, including allergies, problems, medications, and immunization status. Order or execute tests, analyses, diagnostic imaging to inform patient condition. Analyze reports and findings to diagnose conditions. Administer or prescribe treatments, determine medication, dosage, and schedule, and prescribe vaccinations. Provide prenatal and postnatal care to mothers and infants. Perform minor surgical procedures (e.g., toenail removal, mole and cyst removal, foreign‑object removal). Suture wounds. Conduct general orthopedic evaluations and management. Provide basic rehabilitation for musculoskeletal injuries. Offer general internal medicine, family medicine, and pediatrics care. Address general eye problems and interpret ECGs. Provide general gynecological expertise. Communicate with the Medical Doctor regarding quality care and treatment issues. Conduct clinical pertinence chart review studies to assess and enhance patient outcomes. Serve as a resource for community agencies. Use technology tools for research, data entry, record keeping, and business communication via email. Meet all clinical obligations: accreditation, CLIA, drug prescription compliance, HIPAA, state and federal regulations, safety regulations. Follow internal policies and procedures; complete all operational tasks per departmental guidelines. See patients in a timely manner and stay on schedule. Ensure proper coding and appropriate charging for services rendered. Chart in the patient’s medical record within 48 hours of the visit and maintain confidential records. Report to the Director of Primary Care & Quality on operational issues; Director reports to CEO and partner agencies. Administrative Job Functions Demonstrate ongoing support and respect for administration in all communications. Address administrative issues directly and privately. Manage time to see a minimum caseload of 18–20 patients per 8‑hour shift or 23–27 patients per 10‑hour shift. Report directly to the Medical Doctor and the CEO as designated. Complete patient charts and submit medical bills daily. Remain flexible, adapt to change, and model appropriate staff interactions. Comply with all HIPAA rules and regulations. Position has no supervisory responsibilities. Special ADA Requirements Work is performed in a typical interior/clinic environment with normal office noise. Limited physical effort is required. Requires both standing and sitting, with most time spent sitting and using a computer keyboard. Utilizes common clinic supplies: pens, pencils, calculators, keyboards, telephones, printers.
