Home-Based Primary Care Physician — In-Home Patient Care

Bluegrass Care Navigators seeks a Home-Based Primary Care Physician to provide direct patient care in patients’ homes and assisted living facilities in Kentucky. This role also serves as a clinical supervisor for home-based NPs and guides care within established protocols.
Requirements include MD or DO, Kentucky medical licensure or eligibility, DEA registration, and board certification or eligibility within five years.
