Description
Licensed Practical Nurse- Day Shift
Pay Range $29.00–$33.67/hour (based on experience)
Sign-On Bonus: Up to $3,000
Rate : Full-Time, Days
Responsibilities:
- Deliver nursing care under the supervision of an RN in a skilled nursing and rehab setting
- Administer medications and treatments per physician orders and care plans
- Monitor vital signs, symptoms, and changes in condition
- Maintain accurate clinical documentation using electronic medical records (EMR)
- Support post-acute care and recovery goals for short-term rehab residents
- Participate in care plan meetings and collaborate with interdisciplinary teams
- Uphold infection control and resident safety protocols
Qualifications:
Valid LPN
- New graduates encouraged to apply – training available

Prior experience in long-term care, rehabilitation, or skilled nursing preferred
Strong clinical, documentation, and communication skills
