Registered Nurse Case Manager

About the role The Registered Nurse Case Manager plans, organizes, and directs hospice care and is experienced in nursing with an emphasis on community health education and experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Job Responsibilities Patient Care Completes an initial, comprehensive, and ongoing assessment of the patient and family to determine hospice needs, providing a complete physical assessment and history of current and previous illnesses. Provides professional nursing care by utilizing all elements of the nursing process. Assesses and evaluates patient status by writing and initiating a plan of care, regularly re‑evaluating patient and family/caregiver needs, participating in revising the plan of care as necessary, initiating appropriate preventive and rehabilitative nursing procedures, and administering medications and treatments as prescribed by the physician. Uses health assessment data to determine nursing diagnosis and develops a care plan that establishes goals based on nursing diagnosis, incorporating palliative nursing actions and including the patient and the family in the planning process. Counsels the patient and family in meeting nursing and related needs and provides health care instructions as appropriate. Assists the patient with activities of daily living and facilitates the patient’s efforts toward self‑sufficiency and optional comfort care. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility for coordinating patient care for assigned caseload. Communication Completes, maintains, and submits accurate clinical notes regarding patient condition and care given, recording pain/symptom management changes and outcomes as appropriate. Communicates with the physician regarding patient needs and reports changes, obtaining physicians’ orders as required. Communicates with community health‑related persons to coordinate the care plan. Taught patient and family self‑care techniques and provided medication, diet, and other instructions as ordered. Maintains a safe environment for the patient and assists the patient’s family and team members in continuity of care. Collaborates with the interdisciplinary group to meet the emotional needs of patient and family. Attends interdisciplinary group meetings. Additional Duties Participates in on‑call duties as defined by the on‑call policy. Ensures arrangements for equipment and other necessary items and services are available. Supervises ancillary personnel and delegates responsibilities when required. Assumes responsibility for personal growth and development through continuing education and in‑service classes. Fulfills requested or accepted case assignments and participates actively in quality assessment and performance improvement teams. Job Qualifications Graduate of an accredited school of nursing. Preferred: 1–2 years acute care experience in an institutional setting and 1–2 years home care experience. Current licensure in State and CPR certification. Bachelor’s degree, with 1 year of hospice care experience preferred. Management experience not required; responsible for supervising hospice aides. Excellent observation, verbal and written communication, problem‑solving, basic math skills, and nursing skills per competency checklist. Physical ability to walk, stand, lift, position, transfer patients and supplies, reach, stoop, bend, kneel, or crouch; visual acuity and hearing sufficient to perform required nursing skills. Licensed driver with an insured automobile in good working order.
