LPN - PRIMARY CARE - ZELIENOPLE

Physical Condition Performs direct patient care activities such as lab tests, treatments, and medication administration under provider direction. Performs clinical activities, quality control, and performance improvement initiatives to support the Physician Practice. Provides clerical support and serves as a resource for clinical staff. Essential Job Functions Delivers direct and indirect patient care in accordance with the LPN Practice Act and practice policies. Collects patient health data (vital signs, weight, complaint, pain assessment) and involves patients/families and other providers as needed. Completes treatments and testing ordered by physicians and administers medications/immunizations. Maintains accurate, timely documentation and patient education records. Provides patient education, identifies needs, gives instructions, and documents instruction given. Supports patient safety by using two identifiers, labeling specimens, and following infection control procedures. Performs quality control for point‑of‑care testing, participates in medication reporting, and adheres to regulatory requirements. Demonstrates leadership, problem‑solving, delegation, and acts as a patient advocate and preceptor. Helps maintain department compliance with JCAHO and other agencies by participating in survey readiness. Coordinates communication between patients, physicians, pharmacies, specialists, and insurance carriers, including prior authorizations. Assists in performance improvement data collection and implementation of initiatives. Maintains supplies, a clean and safe environment, and handles point‑of‑care testing as ordered. Prepares forms, copies records, and assists with other office functions as required. Performs other duties as assigned. Required Qualifications Graduate of an accredited School of Practical Nursing. Strong leadership, organizational, independent and critical‑thinking skills, sound judgment, and knowledge of legal aspects of nursing practice. Excellent communication skills for diverse audiences. Preferred Qualifications Previous LPN experience. License, Certification & Clearances Current licensure to practice as an LPN in Pennsylvania. Current Basic Life Support (BLS) certification (American Heart Association). Act 33, Act 34, and Act 73 FBI clearance renewed. Supervisory Responsibilities This position has no direct supervisory responsibilities but serves as a coach and mentor for other department positions. Position Type/Expected Hours of Work Scheduled based on operational need (rotate shifts, standby, on‑call).
