The Job Coach works closely with employers, and support teams to ensure that each supported individual receives the accommodations, training, and encouragement needed to thrive and retain their role. - Train the supported individual to complete their assigned job duties up to employer expectations.
- Teach the supported individual appropriate workplace behavior and attitudes.
- Provide mobility training as needed to support access to and participation at the worksite.
- Provide the necessary support on the job to facilitate positive working relations with co-workers, management and the public.
- Attend employee-training seminars at job sites as needed
- Provide supported individual's documentation based on observation of the supported individual's core areas of strengths and barriers to employment: social skills, soft skills, cognitive functioning, and work activities.
- Demonstrated ability to maintain the privacy and confidentiality of supported individuals, including safeguarding personal information and any disclosed diagnoses
- Develop job duty analysis including hourly/daily/weekly task schedule analysis, task breakdown (T.A.) as well as analysis of social and performance skills needed in typical work situations.
- To accurately document hours spent with supported individuals as well as recording detailed progress reports for all time spent with each supported individual.
- To meet with the Supported Employment Supervisor and/or the Supported Employment Manager on a weekly basis to evaluate and report on the progress of each individual. Collect data during training sessions. Intervene in an incident in accordance with the written support plan.
- Train and assist other staff in the required duties of a Job Coach if needed.
- To provide accurate billing information and progress reports by the last day of each month.
- Advocate with employers on behalf of supported individuals when requested and authorized by the individual. (strengthens person-center compliance)
- To assist the Supported Employment Supervisor in the development of the Individual Habilitation Support Plan (IHSP) and in implementing the goals and objectives as defined in the IHSP.
- To carry out other duties as requested by the Supported Employment Manager and Director of Program.
- Ensure that all incidents relating to the Health and Safety of the supported individuals are reported to the Supported Employment Supervisor and documented appropriately.
- Be familiar with and abide by JNCS policies and procedures.
- Participate and complete the Initial Training Program (ITP) within 90 days of employment.
- Participate in regular work supervision and other training development opportunities as required/identified with the position.
- Complete all necessary administrative paperwork specified in managing service - mileage sheets/ timecards/ SIRS in a timely manner.
- Participate in JNCS staff meetings when required.
- Maintain professional conduct at all times and work within the multi-disciplinary team to support all persons served towards achieving their goals.
- Maintain professional attire required to maintain the professional image of Jay Nolan Community Services.
- Other duties as assigned.
- Must be 18 years old.
- Must have High School Diploma or GED.
- Minimum 1 year experience in Human Services field is preferred.
- Must have reliable transportation.
- Ability to communicate clearly, concisely and persuasively.
- Ability to use Smart Phone and successfully complete and submit required documents.
- Strong customer, quality and results presentation.
- Ability to interact effectively at all levels and across diverse cultures.
- Ability to be an effective team member.
- Understands organization culture and values.
- Flexibility, adaptability, and patience in meeting the individual's specific requirements.
- CPR and First Aid certification
