LPN / RN – Home Health Field NurseLocation: Chesterfield, VirginiaJob Type: Full-time / Part-timeWork Location: In personSchedule: * Sunday – Thursday: 11:00 PM – 7:00 AM * Friday: 7:00 AM – 7:00 PM * Sunday & Wednesday: 7:00 AM – 3:00 PM or 3:00 PM – 11:00 PM Job Description:Home Health Services of Virginia is seeking reliable and skilled LPNs or RNs to provide high-quality in-home care for patients. Nurses must have experience in the home health setting and be able to work independently while following care plans. Qualifications: * Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license from the Virginia State Board of Nursing. * Experience working in a home care or home health agency preferred. * Strong clinical, communication, and documentation skills. Job Duties: * Provide skilled nursing care according to the patient’s plan of care. * Administer medications and treatments as ordered. * Monitor and document patient condition accurately. * Complete clinical notes in their entirety. * Follow infection control and safety protocols in the home. * Perform additional duties as assigned.
