Physical Therapist

Physical Therapist provides individualized rehabilitation services to patients across all age groups in a collaborative, multidisciplinary setting near Monterey, CA. The role involves assessing patients, developing treatment plans, and delivering evidence-based interventions while documenting progress and coordinating care with other healthcare professionals. Key Responsibilities:

• Evaluate patients and develop individualized treatment plans based on orders and goals.
• Perform comprehensive assessments and administer evidence-based interventions.
• Design care plans that enhance mobility, strength, balance, gait, endurance, and independence.
• Deliver safe, compassionate care across all age groups.
• Maintain accurate, timely documentation of evaluations, sessions, progress, and discharge plans.
• Assign and supervise PTAs and rehabilitation aides to support efficient care.
• Review and update treatment plans for patients served by support staff.
• Educate patients, families, caregivers, and staff on treatment plans, home exercises, and injury prevention.
• Assist in orienting new employees and volunteers.
• Supervise and evaluate PT and PTA students during clinical affiliations.
• Assume Charge Therapist duties as needed and coordinate care with physicians, nurses, case managers, and other rehab professionals.
• Adhere to safety policies, regulatory standards, and professional guidelines, performing additional duties as assigned.

Qualifications:

• Graduate of an accredited Physical Therapy program.
• Current California Physical Therapist License or eligibility for licensure.
• Current BLS (Basic Life Support) certification for Healthcare Providers via American Heart Association.
• Strong knowledge of evidence-based physical therapy practices and patient-centered care.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work independently while collaborating within a multidisciplinary healthcare team.
