We are seeking an experienced and compassionate per diem RN Case Manager to join our hospice team. As a Hospice RN Case Manager, you will plan, organize, and direct hospice care for our patients and their families.

Up to $50 an hour, depending on experience, with weekly pay.

This is an on-the-road position that requires the candidate to have a driver's license and an insured and reliable vehicle. Candidate must be willing to travel throughout Plymouth County and portions of the Cape.

Responsibilities:

• Supervise hospice aides

• Complete comprehensive assessments of patient and family to determine hospice needs

• Provide professional nursing care by utilizing all elements of the nursing process

• Develop and implement individualized care plans

• Collaborate with interdisciplinary team members to ensure comprehensive care

• Provide health care instructions to patients and families

• Counsel patients and families on meeting nursing and related needs

Requirements:

• Graduate of an accredited school of nursing

• Must possess an active and unencumbered RN license in Massachusetts

• Prior hospice experience preferred

• CPR certified
