Float RN

Oak Street Health seeks a Float RN to provide compassionate, high-quality primary care support across multiple NY centers serving adults on Medicare. The Float RN will perform assessments, administer medications, support chronic disease management, provide patient education, and coordinate care within an interdisciplinary team. This role requires flexibility to work at different locations, strong clinical judgment, and excellent communication skills. The RN will champion preventative care, contribute to care plans, ensure documentation accuracy, and help drive improved outcomes for underserved communities in a collaborative, patient-centered environment.
