As a Home Provider, you’ll help create that sense of belonging by opening your home and supporting an individual with intellectual disabilities as they build a life filled with connection, growth, and independence. Position Snapshot Location: Liberal, Kansas (Must own or rent your current home) Supporting: Male client 1 needs a quiet and relaxing living space with a designated area set-up with a TV for him to watch his favorite cartoons (Batman, Superman, Puppy Dog Pals). Able to facilitate his enjoyment of outdoor activities, such as sitting outside and observing the surroundings. Assist him with mobility using his walker. Male client 2 needs a wheel-chair accessible environment. Facilitate his love for playing cards and dominoes. Facilitate his desire to attend church on Sundays and provide opportunities for him to sit outside and listen to music. How You’ll Make a Difference Every individual is unique, but your responsibilities may include: Supporting daily living skills such as cooking, budgeting, cleaning, and decision-making. Assisting with medication administration and medical appointments. Helping someone build connections, participate in their community, and experience a sense of belonging. Providing transportation and support with daily activities as needed. Maintaining documentation and communication with the Mosaic support team. Helping someone build confidence, develop independence, and pursue personal goals. Depending on the individual’s needs, support may also include: Assistance with personal care activities Behavioral support Medical or physical support needs Specialized care and monitoring What Mosaic Provides Because creating belonging starts with having the right support around you, here’s a look at what you can expect: Initial and ongoing training Dedicated support from an experienced team Resources and guidance throughout the matching process Assistance with licensing and program requirements Ongoing partnership and support Requirements Must be at least 18 years of age You must be a current resident of the state of Kansas (due to state funding restrictions). Own/Rent your own home, apartment, condo, or townhouse Active homeowner’s or renter’s insurance Reliable transportation, valid driver’s license, and current vehicle insurance Internet access and basic computer skills Ability to complete required screenings, background checks, and training Experience is preferred but not required.
