RN Care Coordinator
StartDate: ASAP Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY RN Case Manager supporting colorectal surgery patients through pre-op and post-op care coordination.

POSITION DUTIES 

• Coordinate pre-op and post-op patient care
• Schedule surgeries, imaging, and follow-up appointments
• Collaborate with providers and care teams
• Document care in Epic

MINIMUM REQUIRED QUALIFICATIONS 

• Active RN license (MN required)
• 5+ years of RN experience
• Previous RN CM Experience 
• Epic experience required

PREFERRED QUALIFICATIONS 

• Surgical experience preferred
• Med-Surg background preferred
• Colorectal surgery experience a plus

LENGTH OF ASSIGNMENT 12–20 Weeks
SHIFT / HOURS PER WEEK Monday–Friday, Onsite
40 Hours per Week
