Home Health NurseMajor ResponsibilitiesComplete scheduled patient visits and collaborate with the interdisciplinary team.Conduct comprehensive patient assessments and update plans of care as needed.Communicate treatment changes with physicians and the primary registered nurse.Monitor patient response to care and revise interventions accordingly.Coordinate scheduling and case conferences with primary nurse when appropriate.Assists with the discharge planning process according to agency standards and utilizes interagency/system and community resources to ensure continuity of care after discharge.Communicates problems to physicians and primary Registered Nurse, and clearly states plans, actions and goals so that changes are reflected as they occur.Maintain accurate documentation in compliance with all regulations.Follow agency policies and safely perform required patient care activities.Provide age-appropriate care based on patient needs and developmental stage.Minimum Job RequirementsEducationAssociate's degree in nursing.Certification / Registration / LicenseActive RN license in state of practice.Basic Life Support (BLS) – American Heart AssociationValid driver's license and reliable transportation.Experience3-4 years clinical experience.1-2 years in home health.Knowledge / Skills / AbilitiesAbility to work independently and make sound clinical decisionsExcellent communication (written and verbal) and interpersonal skills.Ability to develop rapport and maintain positive, professional relationships with a variety of patients, staff and physicians.Cultural competence and respect for patient diversity.Good organization, prioritization and problem-solving skills.Physical Requirements and Working ConditionsAbility to multi-task with frequent interruptions.Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and suppliesPreferred Job RequirementsPreferred EducationBachelor of Science in Nursing (BSN).
