Position Summary:
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our healthcare team at Leisure Homestead, a leading skilled nursing facility. The RN will be responsible for providing high-quality care to our residents, including the assessment, planning, implementation, and evaluation of nursing care in accordance with facility policies, procedures, and regulatory standards. The ideal candidate will possess strong clinical skills, exceptional communication abilities, and a passion for working with the elderly population.
Key Responsibilities:
Direct Patient Care: Provide comprehensive nursing care to residents by assessing, planning, implementing, and evaluating individualized care plans based on resident needs and physician orders.
Medication Management: Administer prescribed medications and treatments, monitor for adverse effects, and ensure proper documentation of all medications administered.
Collaboration: Work closely with physicians, nursing staff, physical therapists, and other healthcare professionals to provide coordinated, holistic care to residents.
Care Documentation: Accurately and promptly document all aspects of resident care, including assessments, treatments, progress notes, and changes in condition, using electronic medical records (EMR).
Health Monitoring: Monitor and assess residents for changes in physical and mental health conditions, report concerns to the physician, and take appropriate action as needed.
Resident and Family Education: Provide education to residents and their families regarding health conditions, treatment plans, and health maintenance strategies.
Quality Assurance: Participate in quality improvement activities, including audits, care plan reviews, and infection control measures, to ensure the highest standards of care.
Emergency Response: Be prepared to respond to medical emergencies, providing immediate care and coordinating with emergency services as necessary.
Staff Supervision: Supervise and support nursing assistants (CNAs, CMAs), and other staff members, providing guidance and mentorship as required.
Compliance: Ensure all care activities comply with facility policies, state and federal regulations, and nursing best practices.
Qualifications:
Experience with geriatrics and chronic illness care preferred.
Licenses/Certifications:
Current RN license in good standing with the state board of nursing.
CPR and First Aid certification are a plus.
Additional certifications (e.g., wound care, IV therapy) are a plus.
Skills and Abilities:
Strong clinical assessment and critical thinking skills.
Ability to work independently and as part of a collaborative team.
Excellent communication and interpersonal skills.
Compassionate, patient-focused, and adaptable to the unique needs of elderly residents.
Proficiency in electronic health records (EHR) systems.
Ability to maintain a positive, professional demeanor in stressful situations.
Working Conditions:
Full-time/Part-time, with flexible shift options available (including nights, weekends, and holidays as required).
Exposure to infectious diseases and the potential for physical demands, such as lifting and assisting residents with mobility.
The RN will be expected to follow all safety protocols and personal protective equipment (PPE) guidelines.
