Job Summary:
The organization is seeking a Physical Therapist (PT) specialized in Home Health to work in the California area. This role involves providing comprehensive outpatient physical therapy services to adult and geriatric patients within a home health setting. The position is assigned to a facility that supports interdisciplinary collaboration to deliver quality patient care.

Job Details:

• Schedule: Shift times are from 8:00 AM to 4:00 PM, totaling 40 hours per week.
• Required Qualifications: CA Physical Therapist license, at least 1 year of experience as a Home Health PT, valid CA drivers license and auto insurance. Basic Life Support (BLS) certification is preferred.
• Patient Ratios: 5 visits per day, typically serving 4 to 5 patients daily in a home health environment.
• Certifications: BLS.
• Skills: Experience with adaptive devices, gait training, assessments, home exercise programs, patient education, and working with adult and geriatric populations; familiarity with charting systems, and ability to handle diverse clinical cases including orthopedic, neurological, and general musculoskeletal conditions.
• Additional Information: The assignment begins on 07/27/2026, with an orientation period of approximately 40 hours. The candidate must provide valid licensure and certification documentation, two references within the past year, and pass all onboarding requirements within 3-5 days of contract signing. All compliance documents are due 10 days prior to the start date.
• Travel and Submittal Details: Local candidates are accepted per facility guidelines, and licensure must be uploaded through Nursys within the last two weeks. No separation time is required for return staff.
