Case Management Assistant

Actalent is seeking a Case Management Assistant to support discharge planning from a fully remote environment while you reside in Nevada. The role involves coordinating referrals, authorizations, and transportation, as well as documenting care coordination in Epic EHR and Allscripts. You will work with Case Managers, Social Workers, and payers to ensure timely discharge and appropriate coverage.
