Registered Nurse (RN) – Home Health Case Manager

Location: Wicomico, Worcester & Somerset Counties, MD
Schedule: Monday–Friday

We are seeking an experienced Registered Nurse (RN) Case Manager who thrives in an independent, fast-paced environment and is passionate about improving patient outcomes through coordinated care.

Position Details

• Schedule: Monday–Friday
• Guaranteed Hours: No
• On-Call: None
• License Required: Active Maryland RN License or Compact RN License
• Productivity Expectation: 30 points per week
• Mileage Reimbursement: $0.43 per mile
• Territory: Wicomico, Worcester, and Somerset Counties

Responsibilities

As a Home Health RN Case Manager, you will play a critical role in coordinating and delivering high-quality care to patients in their homes. Responsibilities include:

• Complete comprehensive patient assessments and develop individualized plans of care.
• Coordinate care with physicians, therapists, social workers, and interdisciplinary team members.
• Ensure timely implementation of care plans while monitoring patient progress and outcomes.
• Perform OASIS assessments and maintain accurate, compliant documentation.
• Utilize HomeCare HomeBase (HCHB) to document visits and manage patient records.
• Educate patients and caregivers on disease management, medications, and treatment plans.
• Identify barriers to care and coordinate appropriate community resources and support services.
• Monitor for changes in patient condition and adjust care plans as needed.
• Promote health, prevent complications, and advocate for patients throughout the continuum of care.
• Maintain detailed clinical documentation while meeting productivity expectations.

Qualifications

• Active Maryland RN License or Compact RN License required.
• Previous Home Health nursing experience required.
• Strong knowledge of HomeCare HomeBase (HCHB) and OASIS documentation.
• Excellent assessment, communication, and critical thinking skills.
• Ability to work independently while managing a daily patient caseload.
• Reliable transportation and willingness to travel throughout the assigned counties.
