Physical Therapist - Home Health

Schedule

8-hour day shifts only; weekends not required; no on-call duties.

Assignment Details

Patient load: may visit up to 6 patients per day; guaranteed hours may vary.

Requirements

• Minimum of 1 year skilled home health experience required.
• Current BCLS certification required.
• OASIS documentation experience required.
• Clinical skills in orthopedic post-op, joint replacements, neuro rehab, patient/family education, and home exercise programs are necessary.

Responsibilities

• Provide home health rehabilitation services.
• Conduct skilled assessments and develop treatment plans.
• Educate patients and families on home exercise programs and body mechanics.
• Document patient progress accurately in the required systems.

Compliance / Notes

Travelers are expected to have their own vehicle for patient visits. Parking details and additional compliance requirements will be provided during the onboarding process.
