As a Nurse Practitioner specializing in home health, you will complete risk assessments for Medicare Advantage members throughout Calcasieu County, including locations such as Lake Charles, Sulphur, and DeQuincy.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling options tailor hours to your availability
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive annual or post-discharge home health risk assessments for Medicare Advantage members
- Review prior and current medical history, ongoing conditions, and medication history
- Educate patients about their health conditions and medications, encouraging proactive well-being
- Identify care gaps and provide recommendations for follow-up, empowering better patient outcomes
- Focus solely on assessments no required creation of treatment plans or medication prescribing
Qualifications:
- Active Nurse Practitioner license in Louisiana
- Current Basic Life Support (BLS/CPR) certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Minimum of 6 months NP experience preferred
