Home Health Physical Therapist (PT) - Flexible Schedule Job Summary:
We are seeking a licensed Physical Therapist (PT) to provide high-quality, patient-centered care in a home health setting. In this role, you will evaluate, treat, and support patients in improving mobility, reducing pain, and restoring function-all within the comfort of their homes. Key Responsibilities:

We are seeking a licensed Physical Therapist (PT) to provide high-quality, patient-centered care in a home health setting. In this role, you will evaluate, treat, and support patients in improving mobility, reducing pain, and restoring function-all within the comfort of their homes. Key Responsibilities:
- Evaluate patient function and develop individualized treatment plans in collaboration with the physician
- Perform diagnostic and functional assessments (muscle, nerve, joint, and mobility)
- Provide therapeutic interventions to relieve pain and improve mobility and independence
- Educate patients and families on exercises, treatment plans, and use of assistive devices (canes, walkers, wheelchairs, etc.)
- Communicate regularly with the Nurse/Case Manager and interdisciplinary team
- Document patient progress, treatment responses, and changes in condition in a timely manner
- Participate in development and updates of the Plan of Care
- Coordinate outpatient services when needed
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist (PT) license in the State of Michigan
- Minimum of one (1) year of experience preferred (home health or clinical setting)
- Valid driver's license, reliable transportation, and auto insurance
- Ability to meet physical demands including lifting, transferring, and assisting patients
- CPR certification preferred
- Flexible scheduling
