We're seeking a visionary and dynamic leader who is creative in problem solving and contributes new ideas and solutions to the organization to join our team in Integrated Clinical Services. The Community Health Nurse provides comprehensive community and clinic health services directed towards the prevention of disease, the promotion of health, and the clients self care using professional judgment in comprehensive assessment, screening, nursing diagnosis, care planning, counseling, intervention, case management, health information, education, referral and evaluation. Creates a work culture of respect, trust and understanding for the highly diverse populations the Health Department serves in the office and in the community. Every employee shares the responsibility for promptly bringing to the County's attention conduct that interferes with providing a work environment free of illegal discrimination and harassment. This position requires knowledge of and experience with the African American community and culture. - Proactively assesses, plans, implements and evaluates healthcare services that improves and promotes health outcomes.
- Provide regular screening, comprehensive assessment and interventions for on-going health issues (e.g. depression, diabetes, asthma, wound and pain management, and other chronic diseases).
- Assures completion of referrals and coordinates care with other healthcare providers.
- Collaborates with the healthcare team to adapt the plan of care.
- Specifically knows the African American community, historical and current barriers to health services and has knowledge of the traditions, values and customs.
- Assesses clients needs for in-person and telephone response for urgent and non-urgent medical information.
- Provides health education and promoting self management.
- Responds to emergency events as needed.
- Administer medical treatments and medications.
- Assess, diagnose and prescribe treatment according to standing orders and established protocols.
- Acts as a resource for team needs.
- Maintains clinical skills as dictated by the client population and the clinical practice (adult, pediatric, OB, internal medicine).
- Reviews cases with the providers and team as needed. Seeks expert advice on issues for clients and provides input to providers and nurses on client response.
- Completes competencies required for the position.
- Completes documentation and charting as required by policy.
- Specifically knows the African American community, historical and current barriers to health services and has knowledge of the traditions, values and customs.
- Mentors, coaches and offers clinical expertise to team members.
- Delegates the care of individuals and groups of clients to members of the team based on the scope of practice and clinical skills needed by the clients.
- Assures appropriate client follow-up.
- Fosters community partnerships to best meet the needs of the client.
- Specifically knows the African American community, historical and current barriers to health services and has knowledge of the traditions, values and customs.
- Participates and supports the quality improvement process through team, role, and designated specialty work groups.
- This position will incorporate safe work practices with each task performed and actively participate in initiatives designed to promote a safe work environment.

- Training and experience for this journey level, professional classification is an associate's degree in nursing and a license to practice as a Registered Nurse.
- This position requires an advanced knowledge of and experience working with the African American community. This would be demonstrated by understanding and supporting cultural considerations that impact clients, including over-representation of Black, Indigenous and other People of Color in the criminal/legal system and the impact of trauma, including racial trauma. Specifically knows the African American/Black Community, historical and current barriers to mental health services and has knowledge of the traditions, values and customs.
- License to practice as a registered nurse as outlined by Oregon Revised Statutes (ORS) 678.021 (link is external)
- CPR certification required.
