The Clinical Social Worker will report to the department director, with responsibilities that include completing comprehensive patient assessments; coordinating multidisciplinary and inter-hospital care; managing transitional planning (i.e., patient and caregiver education, aftercare arrangements, payer involvement, and high-utilizer interventions); communicating and collaborating with care team members; supporting departmental goals; documenting per department standards; and participating in performance-improvement activities. Individuals earning quick interviews are licensed social workers in New York State with more than two years of acute care experience.
