Nurse Practitioner Value-Based Primary Care in Altamonte Springs, FL making $140k Compensation: * Base: $130,000+ (Flexible based on experience) * Bonus: Up to $10,000 annually * Sign-On Bonus Available Schedule * MondayFriday, 8am5pm * No nights, no weekends * Limited call: 1 week, 34x per year * First call handled by call center * NP is rarely contacted after triage Practice Model * Value-Based Outpatient Primary Care * Geriatric population * Small patient panel * 1216 patients/day * Dedicated Care Team: * MA, Scheduler, Referral Specialist, Social Worker, Pharmacist * Additional specialty support: Endocrinology & Podiatry Role Overview The Nurse Practitioner will provide comprehensive primary care to an older adult population in a value-based model. This role involves clinic-based care with occasional home visits and telehealth support. The NP will manage chronic conditions, develop treatment plans, educate patients, collaborate with an interdisciplinary team, and maintain thorough documentation aligned with risk-adjustment expectations.
