Working closely with providers, team managers, and external agencies, you will ensure every patient’s treatment plan is comprehensive, compliant, and well-supported by their family and social network. You’ll be a connector—building and nurturing relationships with community health, welfare, and social agencies to create creative, effective solutions for our patients’ needs. You will guide families through complex systems, making successful referrals to community resources that provide the practical assistance they need most. You will play a key role in the patient journey by supporting discharge planning, concurrent reviews, and the coordination of essential post‑discharge services like home care. What minimum requirements you’ll need Licensure / Certification / Registration: Social Worker credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred. Education: High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required. Work Experience: 3 years of experience preferred.
