Housing Stability Case Manager

Care Plus NJ

CarePlus NJ, Inc. is seeking a dedicated Case Manager to support the Housing Supportive Services program in Rochelle Park, NJ. You will provide case management for NJ Family Care members at risk of homelessness, focusing on housing stabilization, intake assessments, and resource referrals. The role requires 1–2 years of experience with homeless or high-need populations, a Bachelors degree, and a valid driver's license. #J-18808-Ljbffr
