Licensed Practical Nurse (LPN) - Long Term Care

LPN – Long-Term Care

Part Time (2 shifts one week, 3 shifts the next week + Every Other Weekend/Holiday)

Contract Length: 27 weeks

Requirements:

• NY LPN Licensure
• BLS Certification (AHA or RQI)
• 1 year of experience as an LPN

Roles & Responsibilities:

• Provide direct and indirect skilled nursing care to assigned residents.
• Collect and communicate data relevant to residents' physical and psychosocial condition.
• Collaborate with the nursing team and interdisciplinary team to create and revise care plans.
• Participate in mentoring and orientation of new staff members.
• Ensure adherence to safety protocols and maintain a safe environment for residents.
• Maintain current knowledge and competency in nursing practice through continued education.
