Description: The Clinical Resource Unit is committed to our Nursing Professional Practice Model. Our dedicated nursing staff, nurse leaders, interdisciplinary team members, and physician partners focus on providing specialized medical and nursing care to various inpatient units. Our highly trained staff utilizes the most current technology and equipment. Medical diagnoses include but are not limited to coronary artery disease, angina, myocardial ischemia, myocardial infarction, dysrhythmia, syncope, congestive heart failure, strokes, cancer, cellulitis, post-surgical care, and chest pain. The environment is fast-paced and always evolving.
Responsibilities:
- Perform all responsibilities and duties required by the unit as specific to the scope of service.
- Implement procedures and theories related to the specific area of practice.
- Maintain up-to-date and accurate documentation of nursing care provided to ensure integration of information for use by the healthcare team.
- Complete initial assessment upon patient admission and develop an appropriate plan of care in accordance with unit standards based on patient and family needs.
- Implement plan of care, nursing interventions, and patient care procedures.
- Modify plan of care based on continuous evaluation.
- Take responsibility for patient and employee safety by reporting and/or correcting safety hazards and incidents.
- Communicate with others directly and privately when necessary to resolve issues.
- Offer constructive feedback to assist in the professional development of peers.
- Confer with colleagues to expand knowledge base.
- Identify own learning needs and take initiative to begin appropriate solutions.
- Fulfill all requirements of departmental competencies.
- Float to various inpatient units including Medical-Oncology, Surgical-Ortho, Cardiovascular Care Unit, Progressive Care Unit, Mental Health Unit, and Acute Rehab.
- Provide direct nursing care and collaborate with multidisciplinary team members to deliver patient care to various patient populations.
- Conduct and document assessments, nursing diagnoses, plans, implements, and evaluates patient care.
- Act as a resource to staff from various units.
- Coordinate care with multidisciplinary team members to facilitate the highest possible quality patient care.
- Manage care of acute and chronically ill cardiac and pulmonary patients.
- Collaboratively identify patient goals of care and design care plans with patients and families.
- Implement interventions as needed including medication administration and evaluation of effects.
- Prepare patients for procedures, manage pain, perform IV venipuncture, blood administration, and monitor labs.
- Discharge patients and provide instruction for safe transition from hospital.
- Document all care using electronic medical records.
- Ensure team huddles are occurring and attend them.
- Delegate with appropriate follow-up.
- Answer call lights promptly.
- Ensure basic care such as turning, toileting, etc., is done per standards of care.
- Handle conflict management and maintain professional communication with all team members.
- Provide post-operative care for the complex surgical population.
Required Skills & Experience:
- Ability to float to multiple units and provide care to diverse patient populations.
- Ability to manage care for both acute and chronically ill cardiac and pulmonary patients.
- Ability to develop, implement, and modify patient care plans.
- Skill in medication administration and evaluating medication effects.
- Skill in preparing patients for procedures and managing post-operative care.
- Skill in IV venipuncture and blood administration.
- Ability to document care accurately using electronic medical records.
- Ability to educate patients and families on care plans and discharge instructions.
- Ability to provide constructive feedback and assist in the professional development of peers.
- Ability to handle conflict management and maintain professional communication with team members.
- Skill in critical thinking to assess patient status and take appropriate action.
- Ability to collaborate effectively with interdisciplinary team members.
- Ability to prioritize patient safety and adhere to all safety standards including fall precautions, pressure ulcer prevention, and ambulation safety.
- Ability to complete patient bedside shift reports, perform hourly rounding, and maintain updated patient communication boards.
- Ability to delegate appropriately and follow up on delegated tasks.
Additional Skills:
- Delegate patient care tasks and validate completion of delegated tasks throughout the shift.
- Complete and document basic RN patient care activities.
- Provide patient medications utilizing the patient 5 Rights: Right Patient, Right Medication, Right Route, Right Dose, and Right Time.
- Maintain patient safety as the highest priority by ensuring compliance with safety measures.
- Maintain a positive attitude that promotes a healthy work environment and healing patient environment.
- Keep a diligent focus on prompt answering of patient call lights.
- Use critical thinking to evaluate overall patient condition and use nursing judgment to identify and act on changes
