The Director of Clinical Quality Performance will be responsible for executing Medrina's clinical quality programs and driving day-to-day performance across hospitalists, SNF, and post-acute services. Reporting to the Vice President of Clinical Quality, this role translates quality strategy into actionable workflows, supports provider engagement, and drives measurable improvement across key initiatives including length of stay, readmissions, transitions of care, and post-acute outcomes. The Director also supports value-based care and ACO program execution through performance monitoring and cross-functional collaboration.

This is a full-time role operating from central or eastern regions of United States during standard business hours Monday - Friday (from your home office remotely) with as-needed travel in United States.  This is not a flexible schedule opportunity. The candidate must reside in central or eastern region of United States.    This role offers an a nnual salary of $160,000 as well as eligibility for an annual performance bonus of up to 10%.  We offer robust benefits package including 15 days of vacation, 7 paid holidays, and 5 sick days annually. Essential Duties and Responsibilities

Below are typical responsibilities and not an exhaustive list of all duties; other duties may be assigned as needed.

1. Executes clinical quality initiatives developed by the Vice President of Clinical Quality, focused on length of stay (LOS), readmissions, care transitions, and post-acute performance across service lines.

2. Implements and operationalizes established clinical playbooks - including Hospitalist, Post-Acute, and Transition Playbooks, ensuring consistent adoption across providers, facilities, and markets.

3. Monitors provider- and facility-level performance against established benchmarks; identifies gaps and implements targeted improvement actions within the existing quality framework.

4. Supports root-cause analyses and ongoing performance reviews in collaboration with the VP of Clinical Quality and Medical Directors; escalates trends and risks appropriately.

5. Executes the physician engagement model established by the VP of Clinical Quality, including facilitating peer-to-peer feedback, distributing trend data, and following up on identified performance gaps.

6. Partners with providers and operational leaders to reinforce adherence to clinical playbooks and quality expectations through structured communication and education.

7. Monitors and distributes provider-facing and operational quality/coding reports developed in partnership with the VP of Clinical Quality and Business Intelligence; ensures accurate and timely delivery to appropriate stakeholders.

8. Utilizes internal and external data sources to track performance trends across physicians, facilities, and markets; translates findings into clear, actionable insights for frontline providers and operational leaders.

9. Maintains day-to-day visibility into SNF and post-acute performance, including facility variation, transitions of care, and playbook adherence; partners with facility stakeholders to address performance issues.

10. Supports execution of ACO and value-based care initiatives by monitoring assigned quality metrics, identifying care gaps/coding gaps at the provider and facility level, and implementing improvement actions under the direction of the VP of Clinical Quality.

11. Tracks assigned deliverables, monitors progress against quality program goals and escalates risks or gaps to the Vice President of Clinical Quality.

12. Maintains working knowledge of CMS programs (e.g., MSSP, LEAD) and regulatory standards sufficient to support performance execution and ensure alignment with compliance requirements.

13. Collaborates with operations, medical leadership, business intelligence, and compliance teams to ensure coordinated execution of quality initiatives.

14. Serves as the operational liaison between quality strategy set by the VP of Clinical Quality and frontline providers and operational teams.

Education and Experience Requirements

1. Bachelor's degree in a related field required; Master's degree preferred.

2. Clinical background preferred (active or previous RN license or equivalent licensure).

3. 4+ years of experience in healthcare quality, clinical operations, or performance improvement.

4. 8+ years leadership experience and exemplary tactics in motivation, cultural awareness, active engagement, relationship building, sound judgment, and the management of sensitive information and situations, development of strategy for quality metric performance improvement action plans and collaborating and presenting to client leadership teams.

5. Experience in hospitalist, SNF/post-acute, or transitional care environments strongly preferred.

6. Demonstrated experience executing value-based care or ACO program initiatives- including HCC/ICD-10 coding, AWV, etc.

7. Experience utilizing clinical informatics and electronic health record systems to support quality performance monitoring and reporting.

Job Requirements

1. Demonstrated ability to execute and operationalize quality and performance improvement initiatives in complex, multi-site healthcare environments; experienced in adapting to changing priorities and work responsibilities.

2.  Working knowledge of CMS value-based care programs, including MSSP and upcoming LEAD, with emphasis on performance execution rather than program design.

3.    Proficiency with electronic health records (EHR/EMR) and data reporting tools to facilitate performance monitoring and operational quality management.

4. Strong analytical skills with the ability to interpret clinical and operational data and translate findings into actionable improvement plans at the provider and facility level.

5. Excellent communication and collaboration skills, including the ability to work effectively with providers, operational leaders, and executive stakeholders; ability to manage difficult conversations and drive alignment across teams.

6. Proficient organizational and project management skills with demonstrated ability to manage multiple priorities simultaneously and drive accountability across cross-functional teams.

7. Ability to deliver training, provide coaching, and monitor compliance with established clinical quality standards and workflows.

8. Enforces compliance with HIPAA and PHI requirements in all quality operations and reporting activities.

Physical Requirements:

• This is a remote, work from home position which handles sensitive, and confidential information. Work must be performed in a secure and private setting.
• Communicating via virtual online meetings daily with reliable internet connectivity with camera on as well as being responsive in a timely manner during work hours via email, MS Teams and phone are required.
• Quarterly travel in United States to on-site visits and company meetings is required.
• Ability to sit, stand, and walk for extended periods.
• Ability to lift and move up to 25 pounds.
