Physical Therapist (PT)

Role Overview: The Physical Therapist provides direct rehabilitation care in physical therapy to patients in a skilled nursing facility setting, focusing on long‑term care and short‑term rehabilitation. Key Responsibilities Develop, implement, and modify treatment plans based on physician referrals and patient needs. Assess patient progress, establish goals, and oversee therapy provided by PTAs, rehab aides, and students. Chart patient data accurately and utilize therapy software effectively. Collaborate with interdisciplinary teams, families, and support staff to ensure comprehensive patient care. Maintain patient safety, confidentiality, and adhere to all regulations and company policies. Participate in community health initiatives and uphold professional standards and conduct. Qualifications & Skills Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT). Active licensure in the applicable state, maintained in good standing. Preferred experience in post‑acute care or related settings. CPR certification (current or obtained during orientation). Proficiency in Microsoft Office and strong communication skills. Knowledge of laws, regulations, and best practices in post‑acute care. Essential Functions: Follow treatment plans, evaluate patient progress, ensure proper documentation, and participate as a team member. Maintain professionalism, demonstrate good judgment, and provide excellent patient service. Physical stamina for patient mobility activities is required.
