Providing great quality patient care is the focus of the RN case manager position. Provide direct care to patients in their homes with a vast variety of medical and surgical diagnoses, using all of your nursing and critical thinking skills. You will collaborate with area physicians and providers, as well as the interdisciplinary team to facilitate patient and family learning and knowledge of the disease process, elements of self-care and promote independence. The case manager creates individualized treatment plan and implements interventions to attain goals of care to make a meaningful impact in the community. Being able to work one on one with patients and families in their homes and follow the progression of the treatment plan is one of the most rewarding settings for nursing care. A comprehensive orientation program is provided as well as in-servicing through the year to keep your nursing skills fresh and up to date.

To excel as a Full-Time Registered Nurse Case Manager at Stratford VNA, you need at least one year of RN experience and a valid State of Connecticut RN license. Applicants need strong organizational and communication skills, knowledge of disease management and have an aptness for teaching patients how to manage their illnesses. The ability to develop a good working rapport with patients, develop trust and work well in an interdisciplinary team are crucial in this role. Applicants must be able to present themselves in a professional manner and have an interest in community health. Home healthcare expereince is preferred and knowledge of the Matrixcare EMR is a plus.
