Behavioral Health Technician - Probation & ReEntry Services

Reporting to the BTG ReEntry and Aftercare Supervisor, the ideal individual is flexible, compassionate, and professional. This role provides direct care services (personal assistance and independent living skills training) and supervision of the patient in the own home. Location: 3864 N 27th Ave, Phoenix, AZ 85017 and 222 E Javelina Ave, Mesa, AZ 85210 Position Full-time Behavioral Health Technician for the Bridging the Gap ReEntry Program in Phoenix, AZ. Schedule: Full-time, Flexible Scheduling, 40 Hours per Week. Responsibilities Direct face-to-face community-based services to assist clients re-entering into the community from jail and prison, ensuring linkage into community services and resources. In-reach into jail, prison, sober living housing and treatment programs to engage members into BTG Re-Entry / TIP services; orient potential clients to the BTG Re-Entry / TIP program. Meet Service Delivery expectations including $2150 weekly productivity, compliance, and timely filing (notes completed by midnight the same day). Provide transportation for members as appropriate and as medically necessary, as determined by the Re-Entry/TIP team, using a personal vehicle or Terros Health provided vehicle when available. Coordinate care with a member’s family, behavioral and general medical providers, community resources, and other involved supports; assist members with connection to physical health services and medical appointment follow-up. Deliver excellent customer service to members, internal and external provider agencies, state entities, or other stakeholders as needed (e.g., probation officers, justice liaisons, advocates, family members, residential staff, hospital staff, vocational rehabilitation). Offer direct care wellness management and recovery strategies using strength-based engagement and psycho-social/educational/rehabilitation services as needed.
