We are seeking a compassionate and experienced Registered Nurse (RN) to join our growing home care team as a Case Manager. In this role, you will have the opportunity to make a direct impact by providing oversight and support to clients receiving care in the comfort of their own homes. This is an ideal position for an RN who enjoys independence, flexible scheduling, and building meaningful relationships with clients and caregivers.

What You'll Do:

• Conduct in-home client assessments and supervisory visits
• Develop, implement, and update individualized care plans
• Supervise and support CNAs and caregivers in the field
• Ensure high-quality, safe, and compliant care delivery
• Collaborate with families and interdisciplinary teams
• Address client concerns with professionalism and compassion
• Maintain accurate and timely documentation

What We're Looking For:

• Active RN license in Maryland
• At least 1 year of nursing experience (home care or case management preferred)
• Strong assessment and care planning skills
• Ability to work independently and manage a flexible caseload
• Excellent communication and leadership abilities
• Reliable transportation to travel to client homes
• CPR certification

Compensation: $70.00 per hour
