A mission-driven community health organization in Northern Oregon, near the Washington border , is seeking a full-time Family Nurse Practitioner or Physician Assistant to join its primary care team.

 Job Responsibilities

In this role, you will:

• Provide comprehensive family medicine services to an established patient panel.
• Evaluate, diagnose, and manage acute and chronic conditions.
• Perform physical exams, preventive visits, wellness care, and follow-up appointments.
• Order and interpret diagnostic testing as clinically appropriate.
• Prescribe medications and coordinate referrals when needed.
• Provide patient education focused on prevention, wellness, chronic disease management, and care plan adherence.
• Collaborate with experienced clinicians and care team members in a community health setting.
• Support access to care for a diverse patient population.
• Complete clinical documentation accurately and timely within the EMR.
• Participate in asynchronous clinical care time as part of the full-time schedule.

Schedule

• Full-time position.
• 4 days per week.
• 32 patient-contact hours per week.
• 8 hours of clinical asynchronous care time per week.
• Outpatient primary care setting.

Minimum Qualifications

• Licensed Family Nurse Practitioner or Physician Assistant.
• Current Oregon NP or PA license.
• DEA licensure in good standing.
• Minimum of 1 year of professional clinical experience in family practice or a related field.
• Strong clinical judgment, communication, and documentation skills.
• Comfortable working in a collaborative primary care environment.
• Interest in mission-driven, community-based care.

Preferred Qualifications

• Bilingual English/Spanish preferred.
• Epic EMR experience preferred.
• Current Washington NP or PA license preferred for telehealth visits.

Compensation and Benefits

This position offers:

• Starting base salary of $130,000
