Location: York, PA Schedule: Monday – Friday | 8:00 AM – 4:00 PM Pay Rate: $16/hour Pyramid Healthcare in York is on a mission to provide exceptional, client-centered treatment for adults facing Substance Use Disorders. As a recognized leader in addiction treatment in York County and across Pennsylvania, we pride ourselves on our passionate leadership team and dedicated staff. We are excited to announce an opening for a Case Coordinator who will be instrumental in coordinating aftercare appointments, maintaining communication with referral sources, and keeping legal or external agencies informed of our clients' progress.

KEY RESPONSIBILITIES:

* Ensure clients have access to the full continuum of care by assessing their needs and facilitating appropriate services.

• Coordinate all appointments, aftercare plans, schedules, and meetings.
• Arrange Halfway House interviews and placements.
• Obtain necessary information and releases from clients, families, referral

sources, and other agencies. * Perform other duties as assigned.

QUALIFICATIONS:
