About the Position

Job Title

Physical Therapist (PT) – Home Health

Location

Valley City, ND

Schedule / Job Type

Contract | Days | Monday – Friday, 8:00 AM – 5:00 PM | No weekend or holiday requirements

Key Responsibilities

• Provide physical therapy services to home health and hospice patients in a home health setting
• Treat approximately 5–7 patients per day across an assigned geographic coverage area
• Evaluate and treat patients with diagnoses including post-surgical recovery, fall-related injuries, and related conditions
• Document patient care and progress using the HomeCare/HomeBase EMR system
• Work independently without on-site PT manager or supervisor oversight during shifts

Required Qualifications

• Active North Dakota Physical Therapy License
• Current BLS certification
• Minimum 1 year of physical therapy experience
• Previous home health experience required
• Valid driver's license and current auto insurance required

Preferred Experience / Skills

• Experience working with home health and hospice patient populations
• Familiarity with HomeCare/HomeBase EMR platform
