ED Patient Navigator: Hep C/HIV Testing & Care Linkage

CarePoint Health is looking for a Patient Navigator in Hoboken to assist in the emergency department with testing for hepatitis C, hepatitis B, and HIV. This role requires collaboration with ED staff and providing patient education and linkage to care for those diagnosed. Ideal candidates will have an associate degree in healthcare or public health and knowledge of HIV and Hepatitis diseases. Additional skills in EHR, excellent communication, and bilingual (English/Spanish) abilities are preferred. #J-18808-Ljbffr
