REGIONAL TRAVEL PHYSICAL THERAPIST (PT) - LIBERTY COMMONS OF COLUMBUS COUNTY

We are currently seeking an experienced REGIONAL TRAVEL PHYSICAL THERAPIST Job Summary Responds to a request for service and obtains physician's orders for evaluation. Responsible for supervising/training personnel such as PTA or PT aides. Evaluates patients and establishes a written plan of care with objective, measurable, and functional goals. Responsible for planning, administering, and supervising the LPTA after the evaluation is complete. Maintains accurate daily records of treatments given to the patient. May be requested to perform facility in-services. Maintains a patient load compatible with the work setting and company standards, as well as maintain a productivity rate of at least 75%, or higher productivity rate as deemed necessary. Responsible for establishing restorative and/or maintenance programs. Assesses treatments and modifies the plan of care as indicated. Maintains weekly progress notes and follows all documentation guidelines. Ensures that all documentation is complete, accurate, and timely. Responsible for preparing and maintaining the work setting, equipment, and supplies. Communicates with the referring physician and any other rehab team member regarding the patient's total treatment program. Follows all established Policies and Procedures of Liberty Long Term Care. Submits all employee paperwork to corporate in a timely manner. Willing to travel to assigned locations, which may include overnight stays. Job Requirements Must be a graduate of an accredited Physical Therapy program. SNF experience preferred. Must have a high level of cognitive and perceptual skills.
