Rockaway Home Care Home Care Intake Coordinator/Specialist Salary: $22.00/hr - $28.00/hr (DOE) Location: Inwood, NY About Us Rockaway Home Care is a licensed home care agency serving the NY Metropolitan area, including Long Island, Rockland, and Westchester counties. We’re seeking an experienced Intake Coordinator to join our team in our Inwood office and support our continued growth in one of the fastest-growing healthcare industries. Role Overview The Home Care Intake Coordinator is responsible for processing patient referrals and guiding patients and families through a smooth start-of-care experience. You’ll contact patients to explain our services, gather required information, and help coordinate their home care needs from discharge through long-term care enrollment, keeping everyone informed throughout the process. The ideal candidate understands home care regulations and processes related to Medicare, Medicaid, and MLTC insurance, and is committed to delivering excellent patient care and customer service. This role is a strong fit for someone who’s motivated, organized, able to work independently, and also enjoys being part of a supportive, team-based intake environment. REQUIREMENTS One year of experience in home care intake Ability to communicate effectively with employees, patients and families, and medical/community affiliates to build positive relationships Computer-efficient and comfortable learning new systems Investigative ability; highly organized; self-motivated; takes initiative Excellent written/verbal communication and strong analytical/critical thinking skills Able to perform with minimal supervision Understanding of the requirements for MLTCs Organized and punctual JOB RESPONSIBILITIES Receive referrals from referral sources Verify patient insurance to determine eligibility Educate patients on the services we provide Send referrals to MLTC for potential enrollment or Medicaid assistance Determine which services were approved by insurance Ensure Rockaway Home Care receives authorization for MLTC cases and provide case specifics to the Coordination Department BENEFITS PTO Sick days Paid legal holidays Health, dental, and vision insurance offered Flexible Spending Account (FSA) Weekly Direct Deposit Daily Pay Available #J-18808-Ljbffr
