Summary The Nurse Care Manager serves as the dedicated clinical care coordinator for the Baylor Longevity Health Program, partnering closely with the Executive Medical Director to deliver highly personalized, proactive care management for a defined panel of longevity health members. This role coordinates advanced diagnostics, preventive care planning, specialty collaboration, and ongoing health monitoring to support optimal health outcomes and longevity. The Nurse Care Manager acts as the primary clinical point of contact for members, ensuring seamless care coordination, timely communication, and a high-touch, concierge-level patient experience. The Nurse Care Manager supports continuity of care across all services, acting as an advocate for members and facilitating access to appropriate resources to optimize overall health outcomes. It does not manage staff. This role supports a premium concierge-style longevity medicine program designed to deliver highly personalized, proactive care focused on optimizing long-term health outcomes. The Nurse Care Manager will work closely with executive leadership, physicians, and interdisciplinary care teams to coordinate advanced diagnostics, specialty consultations, and individualized care planning for a defined member panel. The position requires strong clinical judgment, exceptional communication skills, and the ability to manage complex care coordination in a high-touch, patient-centered environment. This position is located at our McNair Campus. Job Duties Conducts comprehensive patient assessments and coordinate individualized care plans for a defined panel of Longevity Health Program members, focusing on preventive care, chronic disease risk reduction, and long-term health optimization. Collaborates with the Executive Medical Director and interdisciplinary care team to implement individualized longevity health plans, including coordinating advanced diagnostics, specialty consultations, and ongoing monitoring. Facilitates advanced diagnostic testing and monitoring initiatives such as genomics, metabolomics, microbiome analysis, and biologic age testing as outlined in the Longevity program framework. Monitors patient progress through longitudinal health data, including wearable health technology data (e.g., continuous glucose monitoring, sleep and activity tracking), and communicate relevant trends to the care team. Coordinates patient transitions of care, specialty referrals, and interdisciplinary communication to ensure continuity of care across the healthcare system. Provides patient and family education regarding preventive care strategies, risk reduction, and disease management to support long-term health optimization and adherence to individualized care plans. Serves as the primary clinical coordinator for the Baylor Longevity Health Program, supporting a defined panel of members receiving concierge-style preventive and precision medicine services. Coordinates comprehensive longevity evaluations, including scheduling and tracking advanced diagnostics, specialty consultations, and interdisciplinary advisory input. Supports development and implementation of individualized longevity health plans in partnership with the Executive Medical Director and program leadership. Monitors patient engagement and adherence to recommended longevity pathways including cardiovascular health, metabolic health, cognitive health, sleep optimization, nutrition, and physical performance. Performs other duties as assigned. Minimum Qualifications Bachelor's degree in Nursing. Four years of relevant experience. Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing. Current BLS certification from the American Heart Association.
