Halcyon Home Care LLC is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team in Alpharetta, GA. Responsibilities:
- Provide direct patient care including wound care, medication administration, and vital sign monitoring
- Develop and follow individualized patient care plans in coordination with RNs and physicians
- Observe and document patient conditions, progress, and any changes in health status
- Educate patients and family members on care procedures, medications, and health management
- Administer IV therapy, catheter care, and other skilled nursing procedures as ordered
- Communicate effectively with the interdisciplinary care team to ensure continuity of care
- Maintain accurate and timely clinical documentation in compliance with home health regulations
Requirements:
- Active LPN license in the state of Georgia (required)
- Minimum 1 year of nursing experience, preferably in home health or a clinical setting
- Strong knowledge of skilled nursing procedures and patient care protocols
- Excellent communication, time management, and organizational skills
- Ability to work independently and exercise sound clinical judgment
- Valid driver's license and reliable transportation for home visits
- CPR/BLS certification (required or willingness to obtain)
