Job Details: Job Title: RN Home Health Schedule: Monday–Friday, 8:00 AM–4:30 PM; on-call 2–3 nights/week and 1 weekend every 3–4 weeks Location: Warren, Edmonson, Hart & Butler Counties, KY Float / Coverage Area: Four-county service area Job Type: Local contract Contract Length: 13 Weeks Mileage Reimbursement Rate (if applicable) : $0.48/ mile *Pay offered is based on experience, expertise, credentialing, and education. Duties: Perform comprehensive OASIS assessments , admissions, recertifications, and other required home health visits. Provide routine skilled nursing care and monitor patients' conditions throughout the course of treatment. Develop and implement individualized plans of care and communicate changes in patient status to physicians and the interdisciplinary care team. Educate patients and caregivers on medications, disease management, safety, and home care needs. Complete timely clinical documentation while effectively managing a daily field schedule across the assigned service area. Requirements: Education: Nursing degree from an accredited nursing program License(s): Active RN License; must be authorized to practice in Kentucky Certification(s): Active RN License Years of experience: Previous home health experience required Skills/Specialties/Technology: OASIS assessments, skilled nursing, patient assessment, care planning, documentation, time management, care coordination, and ability to work independently in a home health setting Interested?
