ACT Team Case Manager | Certified Mental Health Specialist

Join a dedicated Assertive Community Treatment (ACT) Team that supports individuals with severe and persistent mental illness whose needs extend beyond traditional clinic services. This role involves outreach, enrollment, and providing comprehensive mental health and social support to help clients secure and maintain housing and reintegrate into fulfilling community lives.
Key Responsibilities:- Deliver case management using trauma-focused, person-centered, evidence-based practices to improve emotional and functional well-being.
- Collaborate with the ACT Team to develop crisis prevention plans and individualized treatment goals.
- Coordinate care with community partners, caregivers, medical providers, and other treatment professionals.
- Support clients in achieving treatment goals, medication education, crisis management, and daily living activities.
- Assist clients in attending mental health, medical, and social appointments.
- Complete discharge planning and facilitate transitions to community or higher levels of care.
- Maintain timely and accurate documentation and participate in continuous quality improvement efforts.
- Provide varied schedule coverage including some evenings, weekends, and on-call support as needed.
- Bachelor's degree or equivalent experience in a related field required.
- Preferred degrees include Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics.
- Three to five years of professional experience in mental health services, crisis response, or related fields preferred.
- At least two years of experience educating and supporting clients in self-care and daily living activities.
- Completion of CPST Case Management online training, KHS online training, and background checks including KBI, child abuse, adult abuse registry, and motor vehicle screening.
- Completion of Adult Case Management training within 60 days of employment.
- Valid driver's license, insurance, and reliable vehicle.
- Integrity, compassion, accountability, respect, and excellence in communication and leadership.
- Ability to manage conflict, inspire others, and maintain positive relationships with diverse populations.
