RN Case Management
Seven Healthcare are seeking a dedicated Level 2 Registered Nurse Case Manager with expertise in case management for a rewarding travel assignment in New York, NY. The role focuses on delivering exceptional patient care in a home health setting, utilizing skills in EPIC or Netsmart, OASIS, and Telemetry.

Key Responsibilities of the Level 2 Registered Nurse Case Manager:
- Develops and revises plans of treatment and discharge plans with patients, families, and healthcare teams
- Completes and submits OASIS assessments and related documentation
- Processes orders and communicates patient needs and changes to the physician
- Consults with and educates patients and families on disease processes and self-care techniques
- Coordinates and supervises the quality of patient care services provided by the agency
- Participates in agency operational and financial objectives by monitoring quality indicators and productivity
- Contributes to an environment of professional growth and competency
Requirements of the Level 2 Registered Nurse Case Manager:
- Certified Home Health Care Experience required
- Experience with EPIC or Netsmart
- OASIS and Telemetry experience required
- Current RN License (NY License ONLY)
- BLS Certification
- One year of experience as a clinical nurse in a hospital
- Critical thinking and communication skills
- Ability to work independently and manage various assignments
- Current NYS driver's license
