About This Role The Registered Nurse will provide professional nursing care to patients in their homes, focusing on individualized care through assessment, planning, implementation, and evaluation. This role is ideal for nurses seeking a meaningful connection with patients in a home health and hospice setting. Highlights Location: Woodland Park, NJ Facility: Hackensack Meridian Health At Home Schedule: Full-time What You’ll Do Provide professional nursing care to patients in their homes. Assess, develop, implement, and evaluate home nursing care needs. Ensure compliance with physician orders and documentation. Demonstrate holistic care for patients and their significant others. Complete nursing assessments and medication reconciliations. Update care plans within two weeks of changes in patient status. Act as a liaison among physicians, management, and healthcare team members. Supervise para-professional home health aide staff and conduct visits. Maintain awareness of new standards and incorporate them into practice. Complete routine assignments independently and meet productivity requirements. Requirements Education Associate's Degree in Nursing or Diploma Certificate in Nursing required BSN preferred Experience Minimum of six (6) months RN experience or at least 1 year of LPN experience, preferably in home care or hospice Licenses & Certifications Current NJ State Professional Registered Nurse License required AHA Basic Health Care Life Support HCP Certification required IV Certification preferred About Hackensack Meridian Health Hackensack Meridian Health is a leading healthcare provider committed to delivering exceptional patient care.
