Pay & Schedule

• Pay: $22.00–$24.00 per hour
• Schedule: Monday–Friday, 8:00 AM–4:30 PM

Position Summary

As a Community Resource Coordinator, you will work closely with licensed personal care homes to connect residents and facilities with valuable community resources and support services. You'll build partnerships with local organizations, provide guidance on available programs, and help improve access to services for low-income residents.

Responsibilities

• Build and maintain relationships with licensed personal care homes, community agencies, churches, nonprofit organizations, and other local partners.
• Connect personal care homes with community resources and make referrals as needed.
• Visit licensed personal care homes to assess resident and facility needs.
• Assist facilities in accessing available programs and funding, including:
• Supplemental Security Income (SSI)
• Personal Care Home Supplements
• Waiver Programs
• Food Banks
• Other public assistance programs
• Meet quarterly with SSI homes to strengthen partnerships with local social service agencies and community organizations.
• Interview residents, administrators, and business managers to identify needs and available resources.
• Train alongside Licensing Representatives to learn the licensing process.
• Complete all required licensing, COPA, and professional development training.
• Help facilities identify solutions to issues affecting residents.
• Attend regional Risk Management meetings to discuss concerns, share resources, and identify additional community support.
• Deliver presentations and training to licensed facilities, Department of Human Services staff, and community organizations.
• Prepare accurate reports, including weekly activity reports for leadership.
• Maintain a database of community resources and service providers.
• Participate in staff meetings to identify facilities needing additional support.
• Perform other duties as assigned.

Qualifications

• Bachelor's degree in Human Services, Education, Sociology, Public Health, or a related field.
• At least one (1) year of professional experience developing, implementing, or evaluating human services, education, sociology, or public health programs.
• Equivalent combinations of education and relevant professional experience will be considered.
• Knowledge of community resources and programs available to low-income individuals.
• Strong communication and relationship-building skills.
• Excellent problem-solving and critical thinking abilities.
• Ability to work independently with minimal supervision.
• Proficiency with Microsoft Office Suite and general computer applications.
• Strong written communication and report-writing skills.
• Research and organizational skills with attention to detail
