About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum 2 years of experience required. License: Valid RN License required. Certifications: BLS required. Must-Have: - Experience in acute hospital settings - Familiarity with charting systems, specifically Epic - Knowledge of care coordination and discharge planning Description: Coordinates utilization management and resource management. Oversees discharge planning and post-acute care referrals. Facilitates care for acute patient populations across the care continuum. Aims to provide coordinated and integrated care to prevent unnecessary admissions or readmissions. Promotes patient wellness and improved care outcomes. Strives for efficient utilization of health services. Minimizes denials of payment for patients with complex health needs. Responsible for discharge planning coordination and intervention for high-risk patients. Knowledgeable in appeals and denials processes. Familiar with CMS guidelines and determining medical necessity per evidence-based guidelines. Able to conduct continued stay reviews and assess benefits eligibility. Experienced in disability case management and disease management. Understands HIPAA guidelines and coding procedures, including ICD 10 and CPT. Must perform needs assessment and order durable medical equipment as needed. Works collaboratively within interdisciplinary teams to address patient diagnoses. Serves diverse patient age groups including neonates, adolescents, adults, and geriatrics. Engages with special procedures and equipment as required by patient needs. Actively participates in care coordination to enhance overall patient experiences.
