POSITION SUMMARY:

The Licensed Vocational Nurse (LVN) - Outreach provides direct patient care, case management, care coordination, and patient support services in community, field-based, and home-based settings. This position supports outreach initiatives by assisting with patient education, chronic disease management, continuity of care, and linkage of patients to community outreach resources such as food assistance, housing resources, transportation, and other supportive services for high-risk and underserved populations. The LVN works under the supervision of a Registered Nurse and/or provider and collaborates with interdisciplinary teams to improve patient outcomes, reduce barriers to care, and increase access to services.

ESSENTIAL FUNCTIONS:

1. Conducts patient outreach and case management activities through phone calls, home visits, field visits, and community-based interactions to support engagement in care and continuity of services.

2. Interviews patients to obtain health history, assess needs, and identify barriers to care.

3. Performs basic nursing assessments, including vital signs, weight, and other screening measures within LVN scope.

4. Provides patient education on disease management, preventive care, medications, and treatment plans as directed by providers or RN.

5. Assists in care coordination for high-risk patients, including referrals, follow-ups, and tracking patient progress through programs.

6. Documents all patient interactions, assessments, and interventions accurately in the electronic health record (EHR).

7. Administers medications and immunizations within LVN scope and per organizational protocols.

8. Collects laboratory specimens (e.g., blood, urine) and performs CLIA-waived testing as appropriate.

9. Assists providers and care teams with follow-up on abnormal test results and care plans.

10. Participates in outreach clinics, community events, and health education initiatives (e.g., chronic disease programs, vaccine clinics).

11. Conducts home visits and field visits to gather clinical and social information to support care planning and patient engagement.

12. Identifies and reports concerns such as abuse, neglect, or safety risks per policy and regulatory requirements.

13. Maintains infection control standards and follows OSHA and clinic safety guidelines in all settings.

14. Assists with maintaining supplies and equipment for outreach activities.

15. Supports communication between patients, providers, and departments to ensure continuity of care.

16. Tracks and monitors assigned patient panels or outreach lists to ensure timely follow-up and engagement.

17. Performs nursing triage by assessing patient symptoms and clinical needs via phone, electronic communication, or in person; prioritizes care based on acuity using established protocols; provides appropriate clinical guidance and education; coordinates timely access to care with providers or emergency services as needed; and documents all assessments, interventions, and patient interactions accurately in the medical record.

18. Maintains shared responsibility, under the direction of the DON, for the maintenance of the outreach first response cart, including ensuring medications and supplies are properly stocked, organized, secured, and monitored for expiration dates in accordance with organizational policy and regulatory requirements.

19. At the time of hire, each employee will meet with their supervisor to establish measurable goals and objectives required for the position. These measurable objectives will be monitored by the supervisor and reviewed during the introductory period and annual performance evaluations. Measurable goals and objectives may be modified from year to year, and supervisors are responsible for communicating any updates or changes during the performance review process.

20. May serve as back-up to the Wellness Lobby Receptionist as needed to support clinic operations and patient services.

ADDITIONAL RESPONSIBILITIES

  1. Collaborates with interdisciplinary teams to support coordinated patient care.
  2. Participates in quality improvement activities and outreach program development.
  3. Attends required meetings, trainings, and community events.
  4. Assists with data collection, reporting, and program tracking as needed.
  5. Supports special programs (e.g., chronic disease management, grant-funded initiatives) as assigned.
  6. Serves as backup support to clinic functions (e.g., screening desk) as needed.
  7. Maintains compliance with all federal, state, and organizational policies and procedures.
  8. Performs other duties as assigned.

  10. Ability to follow directions and work within LVN scope under appropriate supervision.
EDUCATION AND EXPERIENCE:
  1. Current, active Licensed Vocational Nurse (LVN) license in the state of employment.
  2. Current CPR/BLS certification (or ability to obtain upon hire).
  3. Valid driver's license and ability to travel for outreach and home visits.
  4. Experience in community health, case management, or outpatient care preferred.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. This position is physically and emotionally challenging. There is a high degree of stress. Ability to deal with time constraints and emotional stress and exposure to body fluids and viruses are also essential functions of this job.

Compensation & Benefits:

Hourly Rate: $32.41-33.72, depending on experience
